Welcome to the Department of Public Health Data Submissions Portal. This secure portal allows partners and regulated entities to electronically submit required data, reports, and documents to the Department.

Use this portal to upload information, complete required submissions, and receive confirmation that your data has been successfully received.

Before submitting information through the portal, please make sure you:

  • Select the correct program or submission type
  • Have all required information or files ready for upload
  • Use accepted file formats where applicable

Fungal Disease

Healthcare providers and hospital staff report cases of Histoplasmosis and Blastomycosis to support disease surveillance and public health response.

Mandatory reporters include physicians, hospitals, and clinical laboratories who identify confirmed or suspected cases. Reports must be submitted within 24 hours of diagnosis using the secure online reporting form.

Data collected supports statewide surveillance, enables timely public health interventions, and informs outbreak response planning across Connecticut communities.

Private Wells

Connecticut DPH–certified laboratories submit well water Microbiological and Chem - Rads quality test results for private and semipublic wells.

Only DPH-certified laboratories are authorized to submit test results through this portal. Submissions must include the sample collection date, analysis method, and all required parameter values.

Results are used to ensure compliance with Connecticut drinking water standards and provide property owners with accurate information about their water quality.

Public Drinking Water

Public water systems submit monitoring reports, treatment data, and sampling site plans required under drinking water regulations.

Regulated public water systems are required to submit periodic monitoring reports covering bacteriological, chemical, and radiological parameters as defined in their individual monitoring schedules.

Treatment modification requests and sampling site plan updates must also be submitted here for DPH review and approval prior to implementation.

Planning and DWSRF

Public water systems submit required planning documents and permit applications, including water supply plans and fiscal and asset management plans.

Submissions include Water Supply Plans, Fiscal and Asset Management Plans (FAMPs), and Drinking Water State Revolving Fund (DWSRF) project applications with all required supporting documents.

Applicants seeking DWSRF funding should review eligibility criteria and required attachments before initiating a submission. Contact the Planning section for guidance.
